CDC's Community Health Resources Web-Based Searchable Database

CDC’s Division of Adult and Community Health, in partnership with many other Centers and Divisions within CDC, developed the Community Health Resources searchable database to assist community health partners, coalitions and activists as they navigate CDC’s web-based resources for planning, implementing and evaluating community-level health interventions. Community Health Resources includes a variety of chronic disease, injury and violence prevention, environmental health, and other important community health topics as well as resources developed by CDC such as program guidelines and recommendations, program planning and evaluation guides and communication campaigns. The database includes links to hundreds of useful planning guides, evaluation frameworks, communication materials, behavioral and risk/protective factor data, fact sheets, scientific articles, key reports and state and local program contacts.
