Mission Statement:

The Central Valley Chronic Disease Partnership is a group of individuals from organizations based in the Central Valley who are concerned about the health and wellness of local communities.

In 2003 the California Diabetes Program worked with partners statewide to conduct a Diabetes Public Health System Assessment.  Partners statewide expressed the need for greater collaboration, a centralized resource network, and more training for professionals.  The Central Valley community expressed the need for further collaboration and to convene a group of health care providers and community groups to share information and resources, and to address issues such as the deficient number of health care providers serving the area. 

The Partnership held their first symposium in 2006 called “Diabetes: Crisis in the Central Valley”.  In 2007, the Partnership hosted a second event called, “An Ounce of Prevention = Pound of Cure".  The event was held in Fresno and attended by 370 health care providers who attended either a Medical Office Staff Luncheon or the Health Care Provider Dinner. A total of 30 Central Valley cities and 13 local and national exhibitors participated in the event. The Symposium covered practical approaches and resources to assist providers in working with those who are at risk for pre-diabetes, diabetes and childhood obesity. Speakers and panel members highlighted prevention efforts focusing on healthy eating and active living as well and quality improvement in the health care setting.
